Accreditation and Credit Statement
This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the Carilion Clinic's Continuing Medical Education Program and Virginia Association of Free Clinics. The Carilion Clinic’s CME Program is accredited by the Medical Society of Virginia (MSV) to provide continuing medical education for physicians. Carilion Clinic designates this educational activity for a maximum of 5 AMA PRA Category 1 Credit™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
Physicians may claim up to 5 hours in Type 1 or Type 2 CME on the Virginia Board of Medicine Continued Competency and Assessment Form required for renewal of an active medical license. |

Target Audience
The target audience for this CME Program is physicians. Other medical professionals who serve low-income, underinsured or uninsured individuals are also welcome to attend. |

Learning Objectives
At the conclusion of this program the successful learner should be able to:
- Explain how the health care reform legislation might impact their medical practice and care of low-income patients
- Describe how to integrate mental health and primary care services in a community-based setting
- Identify tools to use to conduct a diabetes intensive care clinic with mid-level pharmacy care
- Relate how to utilize the medical home model of primary care to provide healthcare to rural and impoverished communities
- Devise a clinic preparedness plan to treat the flu, H1N1

What Does It Cost To Attend?
For the CME Program there is a charge of $40 for members of the Virginia Association of Free Clinics and $55 for non-members. If you would like to attend the full VAFC/VRHA joint conference to include Monday and Tuesday’s sessions as well, you can do so for an additional fee. Click here to register. |
